Controlled Medication Questionnaire

You have been requested to have a review of one or more of your medicines which is listed in our controlled medicines policy. The medication may fall into one of the following categories, for example; - Z-drugs- such as zopiclone or zolpidem. - Benzodiazepines- such as temazepam, diazepam or nitrazepam. - Opiates- such as codeine, dihydrocodeine, morphine, oxycodone, buprenorphine or fentanyl. - Gabapentinoids- such as pregabalin or gabapentin.
